Extending specialist weight management care beyond the clinic

Birmingham Women’s and Children’s NHS Foundation Trust uses Evira within its West Midlands Complications of Excess Weight (CEW) service to support children and families between appointments.

Daily home measurements, trend-based feedback and secure messaging give families a way to follow progress while helping the multidisciplinary team identify when additional support may be needed.

The challenge

Supporting families between specialist appointments

The West Midlands CEW service supports children and young people living with severe obesity and weight-related health complications.

Treatment can extend over long periods and involves input from several clinical disciplines. Much of the work families do, however, happens at home between appointments.

For families travelling long distances or struggling to attend regularly in person, the team wanted a way to maintain visibility of progress and provide support when it was most useful rather than waiting until the next scheduled visit.

Working together

Adding a digital layer to the existing pathway

Evira was introduced into the Birmingham CEW pathway in 2024 as part of the NHS England CEW programme.

Children and families measure regularly at home using Evira’s connected device and see progress as a trend against an individualised treatment target rather than as daily weight numbers.

The CEW team can follow the same trajectory remotely and communicate securely with families between appointments.

This gives clinicians another way to reinforce progress, identify changing trajectories and use motivational interviewing during a journey that is rarely linear.

Continuous support around the existing MDT

The clinical team can remotely follow progress and communicate with families through secure messaging.

When support is needed

A changing trajectory can create an opportunity for the team to reach out, reflect with the family and support the next step.

The longer-term trajectory provides additional context for conversations during telephone and face-to-face appointments.

Published service evaluation

Early outcomes from the Birmingham service

Birmingham’s CEW team published an evaluation of its experience with Evira at the 2025 annual meeting of the British Society for Paediatric Endocrinology and Diabetes.

The evaluation also described a four-year-old whose BMI z-score reduced by 0.68 over seven months while receiving support through the service.

These are observational service data and do not establish that Evira caused the reported changes.

The Birmingham team describes Evira as particularly useful for providing remote support between face-to-face contacts and for families who live further from the hospital.

Implementation learnings

What Birmingham learned from putting Evira into practice

Birmingham’s experience also highlights that technology alone is not enough. Successful implementation depends on how digital support is incorporated into the clinical pathway.

Choose the right patients

Clear inclusion criteria help teams identify children and families most likely to benefit from the approach.

Make it part of the pathway

Evira works alongside lifestyle and multidisciplinary treatment rather than as a standalone intervention.

Allocate clinical time

Teams need capacity to review progress, respond to messages and update treatment targets.

The Birmingham team also emphasises that Evira will not suit every family. Ending digital monitoring when it is not helping allows clinicians to focus support appropriately and use other approaches when needed.

Development

From NHS England programme to continued clinical use

Birmingham adopted Evira through NHS England’s CEW innovation programme and has since incorporated the platform into its specialist service.

The experience demonstrates how continuous home monitoring and asynchronous clinical support can sit alongside an established multidisciplinary pathway, extending visibility and contact beyond individual clinic appointments.

A public procurement record confirms a direct Trust award to Evira Ltd for the digital tool, covering 1 April 2026 to 31 March 2027.
